Kenosha Community Health Center is looking for a Case Manager.

POSITION SUMMARY

Position involves coordination of patient care and community services to patients in an integrated care model. Position functions in collaboration with referring practitioner (i.e., primary care practitioners, dentists) and is clinic based.

PRIMARY ACCOUNTABILITIES

  • Timely referral coordination and documentation
  • Satisfactory completion of coordinator functions related to case management

Relationships

  • Develop and maintain favorable internal and external relationships with multiple strategic partners including co-workers, managers, providers and support staff.

  • Uphold and insure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization.

PRIMARY TASKS AND DUTIES

  • Works in conjunction with interdisciplinary team; reports to the Clinic Operations Manager.
  • Identifies Social Determinants of Health (SDOH) needs and provides consultation to identify psychosocial barriers and works to assist patient in resolving those barriers.
  • Referral and brief case management.
  • Coordination and implementation of quality improvement projects where appropriate.
  • Understand and support the mission, goals and agency wide standards of KCHC.
  • Develop and maintain cooperative relationships with KCHC staff and community agencies.

SECONDARY TASKS AND DUTIES

Perform other related duties as required, which include but are not limited to:

  • Project planning and coordination
  • Perform other duties as assigned.

POSITION REQUIREMENTS

Education

  • Associates Degree in a Human Service-related field (Social Work, Sociology, Criminal Justice, Psychology); Bachelor’s degree preferred

Experience

  • Bilingual (Spanish) preferred
  • Paid or volunteer experience in customer service; experience in health/human services preferred

WORK SCHEDULE

Open availability to a minimum 40 hours during clinic operating hours, Monday – Thursday, 7 am – 7 pm, Friday, 7 am – 5 pm, and rotating Saturdays, 7 am – 3 pm.
